All participants, including adults, must have the ANNUAL HEALTH & MEDICAL RECORD FORM, PARTS A & B on file with the Day Camp in order to participate.    Click on the link above, fill out the form, include a copy of your medical insurance card, and bring with you to the first day of camp.  You may e-mail or US Postage mail the Health form to your Camp Director, but they must have the form prior to the first day of camp.  Parts A & B do not need a doctors signature.

NO CAMPER OR ADULT WILL BE ALLOWED TO REMAIN AT CAMP WITHOUT A PROPER HEALTH FORM

After 5/13/2013 a fee of $10.00 will apply to all Sibling Patrol Registrants.

After 5/3/2013 a fee of $10.00 will apply to all Webelos Registrants.
